PMA Health has partnered with HealthMark Group to assist with medical records requests. Expected turnaround time is 2 business days. Requests should be submitted electronically to HealthMark’s Request Manager at: https://requestmanager.healthmark-group.com/
Follow the instructions to log in to HealthMark and then select “Submit a New Records Request”. Complete the required fields to authorize HealthMark to process the request.
Once the request has been processed, you will be notified via email or mail that the records are available for download.

Federal and state laws permit a fee to be charged for the copying of patient records. The cost will be determined after your medical records request has been reviewed.
